Passport FAQ's - Simplified

New Passport: DS-11

Use this form if:

  • You're applying on behalf of someone under the age of 16.

  • It is your first U.S. passport.

  • Your previous U.S. passport was issued over 15 years ago (Your previous U.S. passport expired over 5 years ago).

  • If you are over 16 and were aged 15 or under when you were issued your last passport.

Passport Renewal: DS-82

Use this form if your previous passport:

  • Is in your possession and is undamaged.

  • Was issued when you were aged 16 or older.

  • Has few blank passport pages remaining.

  • Will expire soon or has expired within the last 5 years.

  • Was issued over a year ago and you have since legally changed your name.

Lost or Stolen Passport: DS-64

Use this form if your previous passport:

  • Was lost or stolen and you wish to report the loss.

  • Was lost or stolen and you wish to report the loss AND apply for a new passport.

Passport Changes and Corrections:
DS-5504

Use this form if the following applies:

  • Your passport has a printing error that occurred within a year of its issue date.

  • Your passport is in good condition.

  • You have documentation to support the change or correction you are requesting.

  • You have a “Limited Validity Passport”.

Child Passport: DS-11

Use this form If applying for a new passport OR to renew a passport for a minor under 16, you need:

  • Proof of U.S. citizenship.

  • Proof of parental relationship.

Evidence of U.S. Citizenship:

Valid, undamaged U.S. passport (expired passports are acceptable)

U.S Birth Certificate that meets the following requirements:

  • Issued by the city, county, or state of birth

  • Lists applicant’s full name, date of birth, and place of birth

  • Lists parent(s)’ full names

  • Has the signature of the city, county, or state registrar

  • Has the date filed with registrar's office (must be within one year of birth)

  • Has the seal or stamp of the city, county, or state which issued it

Consular Report of Birth Abroad or Certification of Birth

Certificate of Naturalization 

Certificate of Citizenship

NOTE: Photocopies are not acceptable. Documents must be original. Originals will be mailed back to you.

Valid Photo ID:

You must present a physical, government-issued photo identification document (ID) when submitting Form DS-11 if you are:

  • Over age 18

  • The parent or guardian identifying an applicant under age 18

You must present one physical, primary ID which includes your photo. We cannot accept digital IDs.

  • Valid or expired, undamaged U.S. passport book or passport card 

  • In-state, fully-valid driver's license or enhanced driver's license with photo

  • Certificate of Naturalization 

  • Certificate of Citizenship 

  • Government employee ID (city, county, state, or federal)

  • U.S. military or military dependent ID

  • Current (valid) foreign passport

  • Matricula Consular (Mexican Consular ID) - commonly used by a parent of a U.S. citizen child applicant

  • U.S. Permanent Resident Card (Green Card) - commonly used by a parent of a U.S. citizen child applicant

  • Enhanced Tribal Cards and Native American tribal photo IDs

  • Note: you may be asked to present an additional ID when presenting one of these three documents. 1) In-state, fully-valid learner's permit with photo, 2) In-state, fully-valid non-driver ID with photo, or 3) temporary driver's license with photo.
